呼吸道合胞病毒最早于1955年由美國沃爾特·里德陸軍研究所在患有呼吸道疾病的黑猩猩體內(nèi)發(fā)現(xiàn),1957年Chanock和Finberg從兒童體內(nèi)分離出,由于其特殊的細胞融合性病變,所以命名為人呼吸道合胞病毒(human respiratory syncytial virus,HRSV)。據(jù)報道,HRSV是世界范圍內(nèi)嬰幼兒下呼吸道感染最重要的病原體之一,有些地區(qū)2歲以內(nèi)的兒童至少發(fā)生過一次感染。近年來,研究發(fā)現(xiàn)HRSV也可引起成年人、老年人以及免疫抑制患者下呼吸道感染。2015年全球范圍內(nèi)由HRSV引起的呼吸道感染人數(shù)約3.31億,導(dǎo)致約320萬5歲以下兒童住院治療,其中約6萬名住院死亡,而老年人住院約33.6萬,住院死亡約為1.4萬,醫(yī)院和社區(qū)每年因HRSV死亡人數(shù)約為10萬~15萬。HRSV占全球兒童急性下呼吸道感染的22%,中國HRSV感染率約為31/1000,占兒童急性下呼吸道感染的18.7%。HRSV觸發(fā)宿主炎癥免疫反應(yīng),雖然人體對HRSV免疫反應(yīng)尚不完全,但會引起疾病進一步發(fā)展,且HRSV致病機制也尚不完善。HRSV臨床尚無有效疫苗,開發(fā)有效疫苗和單克隆抗體,即是機遇,也是挑戰(zhàn)。隨著呼吸道合胞病毒多種基因型相繼出現(xiàn),并呈現(xiàn)交替流行以及共流行的現(xiàn)象,基因型的不斷變異、變遷,所造成的疾病程度也有所不同。
