無論是在康復師的角度也好，還是在患者的角度也好，我們會發(fā)現在大部分人在進行半月板或者前交叉韌帶術后的康復都是相近的！幾乎沒有什么區(qū)別！然而這是正確的嗎？

這是錯誤的?。?！

在前交叉韌帶損傷與半月板損傷里對于患者的術后康復方案雖然在一定程度上有很多重合的地方，但是在對于手術部位的保護以及對于運動的選擇上有著不一樣的治療觀點，閱讀本文的意義對于康復師來說在選擇康復方案時能盡可能的規(guī)避風險，對于患者來說減少無用的治療方案，減少治療的時間與金錢，從而達到最佳的治療效果。

在物理治療的范疇里面對于術后康復我們需要注意一些什么

急性前交叉韌帶與半月板損傷機制

保守治療與手術治療的選擇

半月板：

在半月板中臨床的一線治療是保守治療（非甾體抗炎藥，物理治療）為主

根據文獻資料顯示：在退行性半月板撕裂中，物理治療在 5 年的膝關節(jié)功能方面不亞于關節(jié)鏡下部分半月板切除術。物理治療有著不亞于手術治療的治療結果。

在前交叉半月板中大多數人都會推薦進行手術治療

文獻資料顯示：大多數術后膝關節(jié)穩(wěn)定性研究發(fā)現，前交叉韌帶重建術后的膝關節(jié)功能優(yōu)于保守治療。保守治療中因為前交叉韌帶損傷導致約束脛骨前移的力減少，作為二級約束的半月板獲得了更多的壓力，加速了半月板以及軟骨的磨損，增加了關節(jié)炎的幾率

前交叉韌帶重建術后（ACLR）術后早期（1~4周）

對于運動的選擇

半月板：

在半月板損傷中前期的閉鏈訓練是相應的較少的，閉鏈運動會對于修復的半月板產生一定的壓力。

前交叉韌帶：

前交叉韌帶主要控制而對于前交叉韌帶重建來說，開鏈運動對于前交叉移植物的壓力是很大的

開放動力鏈 (OKC) 練習存在爭議，因為它們可能會對移植物造成壓力。OKC 膝關節(jié)伸展練習的初始限制可能從 ROM 開始到 90° 到 45°，以防止移植物損傷，隨著移植物成熟而進展到完全 ROM。

前交叉韌帶的生長期（增殖期）在術后7-21天，韌帶在第5-21天時是最脆弱的

支具的使用

半月板修復支具的使用：

患者進行了半月板修復，則建議穿戴膝關節(jié)支具或膝關節(jié)固定器以防止膝關節(jié)在負重時彎曲，這可能導致修復的半月板再撕裂。膝關節(jié)固定器可使膝關節(jié)完全伸展，而鉸鏈式護膝可以控制膝關節(jié)的運動。

前交叉韌帶支具的使用：

前交叉韌帶重建后伸直固定很有必要，這樣對于新固定的韌帶不會造成壓力，使關節(jié)有時間正常愈合。

對于支具的話，可憑個人意愿購買。研究表明支具沒有多大的治療意義，佩戴膝關節(jié)支具可以減少 ACL 損傷患者的不穩(wěn)定感，但最初使用支具也會導致日常生活活動中出現更多不適。

負重時間

前交叉韌帶：

前交叉韌帶術后康復指南上寫到，ACL重建術后可在耐受的情況下即刻負重。有數據顯示即可負重有利于髕股關節(jié)疼痛的減少， 35% 降至 8%。

半月板：

半月板修復術后康復指南里提到：修復術后的患者在2~6周往往需使用拐杖進行無負重或者部分附中

在損傷部位的撕裂類型上也有不同的負重要求：

外周修復往往只需要2周的時間就可以部分負重在3~4周內就可以在可接受的不適下負重行走。

而復雜性的撕裂則需要更久的時間6周部分負重，7~8周可耐受負重

加速康復方案：

在最新的研究中指出加速康復方案，在可以耐受的情況下盡早完全支撐，無需支具并立即活動手術膝關節(jié)。Shelbourne和 Mariani報告說，在接受加速康復方案和常規(guī)康復計劃的患者中，半月板愈合的百分比很高，但加速組在全范圍運動中表現出更快的恢復（加速組 6 周與標準組相比 10 周），2 個月時股四頭肌強度更高（加速組 82% 與標準組 71%），并且加速恢復完全活動（加速組 10 周與標準組 20 周）標準組）

總結

在兩者的術后物理治療中都強調盡早地進行活動

在對于前交叉韌帶重建術后與半月板縫合康復計劃的統(tǒng)計學研究中顯示早期運動是安全的，有助于避免后期關節(jié)纖維化的問題。對選定的有動機的患者進行最低限度監(jiān)督的物理治療是安全的，沒有明顯的并發(fā)癥風險。

在 4 個月內每周進行 3 次體育鍛煉可使膝關節(jié)功能改善 35% 以上。
